If you're applying for coverage outside the open enrollment period offered, you must complete the Evidence of Insurability form.

Voluntary Term Life
Members wanting to increase the amount of voluntary term life coverage over the guarantee issue limit offered must complete the Evidence of Insurability form.

Enrollment for RLC State Association Employees

For members that are not receiving a paycheck from the postal service but from your state association, LTD and Life insurance premiums will be paid through your personal checking account. Click here for ACH form.

Instructions for P.A.I. D.

  1. Please submit voided check (no deposit slips) and one month’s premium made payable to GIS
  2. Premium will be deducted around the 15th of each month for the next month’s coverage
